INDEPENDENT LIVING

Allows residents to maximize personal freedom while residing in an apartment of their choosing. Resident have an overall strong physical and emotional health, and do not require assistance with activities of daily living.

CONTINUUM OF CARE

ASSISTED LIVING

The team of certified caregivers, medical and nursing professionals, dietary and social coordinators work with residents and family members in designing an individualized and holistic wellness plan for seniors requiring minimal to hands on care. Three different levels of care are available to our residents and the service plan is customized to meet their needs in the privacy of their studio or one bedroom apartment.

We offer an extensive range of services, including:

Bathing, dressing, grooming, and continence support
On site physician services providing labs, x-rays, ekgs
Mobility, transfer, and escort assistance
Medication management and assistance
Dietary and nutritional support
Wellness nursing services
Activities throughout the day
Transportation services

Our nursing team manages and monitors chronic health conditions, such as diabetes, Parkinson’s, stroke, and communicate with your healthcare providers and family. As needed, we coordinate medical appointments and provide transportation.

MEMORY CARE

For residents needing loving and compassionate care due to dementia or Alzheimer’s diagnosis. The secure neighborhood allows residents who wander to enjoy daily activities in a safe and secure environment.

The following services are available in an efficiency studio or companion suites:

Full hands on assistance with bathing, dressing, grooming, ambulation/transfer, and continence management – staff provides complete care for activities of daily living
Medication administration and management – residents might need medications directly administered or crushed
Personalized meals prepared to provide high nutrition and feeding needs, such as pureed foods, snacks through out the day, small cut up food.
A wholistic team approach in coordinating and providing care as the resident progresses through the various levels of care – family and medical professionals collaborate in the administration of daily living activities
Customized social activities geared to the needs of each specific individual
Low staff ratio tailored in meeting optimized levels of attention for our residents

SKILLED NURSING SERVICES & HOSPICE CARE

For residents needing an advanced level of care as a result of a physical or mental disability, such as individuals using a walker, wheelchair or are bed bound, Heritage provides an array of services:

Full care assistance with activities of daily living such in bathing, dressing, grooming
Full medication administration and monitoring
Nursing services related to:
- diabetes,
- blood pressure checks,
- weight monitoring,
- wound care,
- coumadin checks
- breathing treatments
- IV therapy
- G tubes
- Incontinence care
Physical, Occupational and Speech Therapy
Transfer with 1 or 2 staff assistance
Hoyer lift transfers
Turning every 2 hours, and every 30 minutes checks or as required
Individualized one on one care for resident available
High low hospital bed and mattress provided
Assistive devices in feeding or hand on feeding

For residents approaching end of life, they can continue to remain in our community and be provided with nursing services to meet their needs.

Third party Hospice agencies work alongside our team of nursing and medical professionals to coordinate individualized care.
